Health Office

Welcome to the School Nurses’ Health Office. Our goal is to keep students as healthy and safe as possible for an optimal learning experience. If you need to reach the nurse, please call 610-356-6225 ext. 138 or contact the school office.

All new students and students entering Kindergarten and 6 ^th grade must have a physical examination form completed by their doctor prior to the start of school.
 
All students must have the dental form completed by their dentist in the following grades:  Kindergarten, 3 ^rd and 7 ^th

Pennsylvania State Law requires proof of complete immunization for all students (see Health   Topics for details on immunizations.)  Additional immunizations are needed for 7 ^th grade.
