Current Family Registration
This year all existing families will be automatically re-registered for the next school year.
FACTS will automatically charge you the $250 registration fee on January 10, 2025.
If you do not plan on registering your child(ren) for the next school year please email Susan Mills at [email protected] before December 13, 2024.
If you have any new students to add next year please fill in the form below and send back to Susan Mills before December 13, 2024.
Your children cannot be registered if you do not send in a copy of their birth certificate, new student registration form, copy of baptismal form (unless baptized at St. Anastasia) and a copy of their latest physical and immunization forms. A dental form is needed for any new children entering Kindergarten. Please see our website for these forms.

New Family Open House and Registration
Please check our school calendar for any open houses. They are usually held during Catholic Schools Week.
Please stop in and get a tour of our school visit classrooms and see our school in action. Please contact Susan Mills at [email protected] or 610-356-6225 ext. 118 to arrange a tour.
Our new family registration will take place on January 30th between 8 am-10 am. Registration forms will be available at that time.
Please make sure you bring copies of the following necessary documents:- Baptismal Certificate
- Birth Certificate
- Health Form
- Immunization Form
Your child cannot be registered without all the necessary documents.
